EMERGENCY
After Hours Instructions
If you are an active patient at this practice and you are experiencing an emergency or have an urgent concern, please follow the instructions below. Thank you.
For emergencies, please take the following steps:
***PLEASE CHECK YOUR EMAIL COMMUNICATIONS FROM THE PRACTICE FOR ANY EMERGENCY COVERAGE INSTRUCTIONS, INCLUDING PROVIDERS AND CONTACT INFORMATION, IN CASE OF EXTENDED OFFICE CLOSURES***
1. Contact me via the patient portal leaving a detailed message with your urgent concern and your contact information. I will respond to emergencies within 24 hours.
Routine medical concerns are not considered emergencies and will be addressed when the office reopens. These include medication refills, prior authorizations, completion of insurance or other forms, billing issues, medical record requests, etc.
2. If your concern is too urgent to wait for a reply, please seek treatment at the nearest emergency department or call 911/988. Once you are safe, please contact me via the portal and/or phone so that your care can be managed accordingly.
3. If possible, please have the treating facility, a family member, friend, etc. call the office and leave a voicemail to provide a detailed message with your urgent concern.
PLEASE NOTE: Email is NOT an appropriate medium for urgent or emergency messages, it is not a confidential form of communication and it should not be used for sending personal medical information.
REMINDERS:
For Prescriptions:
Please start by calling your pharmacy directly to ask about the status of your refill. Refills may already be available even if your bottle at home shows otherwise; or the prescription may already be received and on file, or in process. You may request that your prescription be transferred from one pharmacy to the pharmacy of your choice.
Refills are provided at each appointment to ensure an adequate supply of medication until your next appointment. As long as regular follow-up is maintained, your prescriptions on file should last until your next scheduled appointment.
Please feel free to send me any other questions or concerns. I will work to reply to any routine medical concerns as promptly as possible once the office reopens.
Thank you very much for your time and attention. As this office is not a crisis facility and I may not be available at all times or after hours in the evening, if you are experiencing an emergency, please contact an emergency service provider or go to the nearest emergency room.
